Sage Health Centre provides clients with comprehensive personalized relapse prevention and continuing care plans that supports clients in developing and maintaining their ongoing addiction recovery.
Clients can also access ongoing professional outpatient support that enhances her/his addiction recovery by attending Sage Health Centre’s virtual continuing care group programs via Zoom at no additional charge.
Relapse Prevention Plan
Prior to discharge, clients complete an in-depth personalized relapse prevention plan that identifies potential trigger/warning signs and appropriate relapse prevention strategies.
Continuing Care Plan
As studies show that maintaining total abstinence is the most effective means of treating addiction and success rates greatly improve with ongoing support, post treatment recommendations include regular follow up with family physicians, continued pro-active health and wellness strategies in order to maintain a healthy balanced lifestyle, participation in additional outpatient group and/or individual outpatient support offered through the Sage Health Centre Continuing Care program and/or local resources for a minimum of six months and regular attendance of 12 step/SMART recovery meetings.
Prior to discharge, clients complete an in-depth personalized continuing care plan that includes researching 12 step/SMART recovery meetings located in their communities; referrals for additional outpatient support that access local medical and mental health and addictions resources located in their communities and return to work requirements.
Distance Program
Sage Health Centre offers a comprehensive outpatient continuing care program to support clients in developing and maintaining self-efficacy in their recoveries. Clients are able to access on ongoing professional support to aid her/his addiction recovery by attending four weekly virtual continuing care groups via Zoom at no additional charge.
Group Programs:
Virtual Alumni Continuing Care Group Program: Sage alumni have unlimited access to our virtual continuing care therapy group that is hosted by clinical counselling staff on Mondays from 5:00 – 6:30PM (PST) via Zoom.
Virtual Alumni 12 Step Meeting: Sage alumni have unlimited access to our virtual 12 step meeting that is hosted by clinical counselling staff on Wednesdays from 5:00 – 6:00PM via Zoom.
Virtual 12 Step Meeting: Sage alumni have unlimited access to our client 12 step meeting that is hosted by an alumni on Saturdays from 10:30 – 11:30AM (PST) via Zoom.
Virtual SMART Meeting: Sage alumni have unlimited access to our client SMART meeting that is hosted by clinical counselling staff on Sundays from 10:00 – 11:00AM (PST) via Zoom.
We encourage clients to attend weekly for a minimum of six months with 1 year to 2 years being the identified beneficial amount of continuing care. Alumni have unlimited ongoing access at no additional charge.
Individual Follow Up
All alumni are encouraged to join Sage Health Centre’s Facebook page; clients are also invited to contact clinical and/or support staff via telephone or email at any time.
